Please contact Zimmer Dental's Customer Service Department prior to returning products.

1. Customers may return UNOPENED and UNDAMAGED Zimmer Dental products in its original packaging only. All returned products must be in saleable condition with the manufacturing seal intact.

2. Returned products must be accompanied by a Customer Return Number obtained from a Customer Service Representative. If products are returned to Zimmer Dental without a Customer Return Number, a 15% restocking fee will be charged.

3. No credit, exchanges or refunds will be provided on discontinued products or products with a shelf life less than six months. Credit or refund for returned items will be issued upon receipt of the goods by Zimmer Dental.

4. All freight must be prepaid on returned merchandise.

5. Customers returning products within 30 days may choose from the following options:

• Full credit
• Full exchange on product.
• Full refund.

6. Customers returning products within 60 days of purchase may choose from the following options:

• Full credit to customer account
• Full exchange on product
• Refund minus 15% restocking fee

7. Customers returning products within 61-90 days of purchase may choose for the following options:

• Credit to customer account minus 15% restocking fee.
• Full exchange on all products.

8. After 90 days only IMPLANT BODIES will be exchanged (up to 120 days after purchase). No other products will be accepted after 90 days.

Contact Zimmer Dental's Customer Service Department at 800-854-7019 to obtain a Customer Return Number. This number must appear on the outside of the shipping box that contains the items being returned to Zimmer Dental. Do not mark on any of the product packages or labeling.
